Annual Emission Report Forms

Annual Emission Report: Illinois EPA Annual Emission Report informaton

Annual Emission Report Rule: This is a link to the Illinois Pollution Control Board's site which includes all regulations by the Agency. You will need to scroll down to Part 254.

Instructions: These are the instructions for all sources that are required to submit an Annual Emission Report.

Address/Source Emissions: This form is used to report company information and source-wide emissions. This form is required to be submitted for all types of annual emission reports.

Combustion Emission Units: This form is used to report data for fuel combustion emission units. It only needs to be completed for sources required to submit the "long" annual emission report form.

Process Emission Units: This form is used to report data for process emission units (reactors, crushers, coaters, etc.). It only needs to be completed for sources required to submit the "long" annual emission report form.

Storage Tanks: This form is used to report data for storage tanks. It only needs to be completed for sources required to submit the "long" annual emission report form.

Other Emission Units: This form is used to report data for emission units that are not combustion units, process units or storage tanks. Typically, emissions units in this category will be fugitive in nature (storage piles, roadways, clean-up solvent, etc.). It only needs to be completed for sources required to submit the "long" annual emission report form.

Emission Unit Emissions: This form is used to report emissions from combustion units, process units, storage tanks or other emission units. For every one of those forms completed, the emissions form must also be completed. It only needs to be completed for sources required to submit the "long" annual emission report form.

Controls: This form is used to report data for control devices and control methods. It only needs to be completed for sources required to submit the "long" annual emission report form.

Stacks: This form is used to report data for stacks and exhaust points. It only needs to be completed for sources required to submit the "long" annual emission report form.

Coating/Solvent Use: This form is used to report data on coating and solvent use. It only needs to be completed for sources required to submit the "long" annual emission report form.

Malfunctions: This form is used to report data on emissions from startups, shutdowns and malfunctions. It only needs to be completed for sources required to submit the "long" annual emission report form.

Having Trouble Opening These Forms?: These forms must be opened using Acrobat Reader. If you click on one of these links and get a message that starts with "Please wait...", then your computer is trying to use the web browser rather than Acrobat Reader to open the file. In that case, do this:

  1. Right-click on the link and select "Save link as..." or "Save target as..." to save the file.
  2. Start Acrobat Reader.
  3. In Acrobat Reader's menu, select "File", then "Open" to open the saved file.

Any questions about Annual Emissions Reports (AERs) should be directed to the email address EPA.AERQuestions@illinois.gov. When sending an e-mail, please make sure to include your name, phone number, e-mail address, the ID number of the source in question, and a detailed description of your problem or question. Depending on the time of year, there may be a large number of questions, so please be patient.
