The Illinois Environmental Protection Agency has issued an Areawide Air Pollution Action Day for Sunday, July 19th. This is in addition to the ongoing Chicago Air Pollu tion Action Day in effect today July 18th. This alert includes all of Illinois’ Air Quality Index (AQI) forecast sectors, except Metro-East St. Louis. Visit www.AirNow.gov to stay up to date on current conditions.

  1. Right-click on the link and select "Save link as..." or "Save target as..." to save the file.
  2. Start Acrobat Reader.
  3. In Acrobat Reader's menu, select "File", then "Open" to open the saved file.

Each form below can be filled out online but not submitted online. Download and fill out form using the Adobe Acrobat Reader. When you are finished entering your information in the form fields, print out the form, sign and date, then mail to Illinois Environmental Protection Agency. Further instructions and addresses are included with each form.
