Storage/Treatment/Transfer of Wastes

Note: These forms are not be used for (1) the storage, treatment or transfer of potentially infectious waste; or (2) storage/transfer of non-special municipal waste. Each of these types of facilities has their own set of forms.

The following form is required for to be submitted along with all permit applications:

• Notice of Application for Permit to Manage Waste (LPC-PA16)

The following application forms are required to obtain the initial developmental permit:

• General Application for Permit (LPC-PA1)
• Application for Permit to Develop Treatment/Storage Facilities (LPC-PA3)
• Certificate of Siting (LPC-PA8) (if needed)
• Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
• 39(i) Certification (for operating a waste management facility)

The following application forms are required for an operating permit:

• General Application for Permit (LPC-PA1)
• Application for Operating Permit (LPC-PA4)

The following forms are required to modify a permit:

• General Application for Permit (LPC-PA1)

The following forms must be submitted to transfer facility ownership/operation to another entity:

• General Application for Permit (LPC-PA1)
• Application for the Transfer of Solid Waste Permits (LPC-PA7)
• Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
• 39(i) Certification ( for operating a waste management facility)

The following forms must be submitted to certify closure of the facility:

• General Application for Permit (LPC-PA1)
• Affidavit for Certification of Closure of Non-Hazardous Transfer, Storage or Treatment Facilities
