Municipal Waste
The following form is required for to be submitted along with all permit applications:
The following application forms are required to obtain the initial developmental permit
- General Application for Permit (LPC-PA1)
- Application for Permit, Non-Special Municipal Waste Storage/Transfer Facility (LPC-PA5)
- Certificate of Siting (LPC-PA8) (if needed)
- Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
- 39(i) Certification (for operating a waste management facility)

The following forms are required to modify a permit
The following forms must be submitted to transfer facility ownership/operation to another entity:
- General Application for Permit (LPC-PA1)
- Application for the Transfer of Solid Waste Permits (LPC-PA7)
- Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
- 39(i) Certification ( for operating a waste management facility)
The following forms must be submitted to certify closure of the facility
