Operating Landfills Under 35 IAC 811
(Landfills operating after September 18, 1992)
The following form is required to be submitted along with all permit applications:
The following forms must be submitted to obtain an initial permit
- General Application for Permit (LPC-PA1)
- Landfill Development Permit Application (LPC-PA2)
- Certificate of Siting Approval (LPC-PA8) (if needed)
- Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
- 39(i) Certification (for operating a waste management facility)
The following forms are required to modify a permit
The following forms must be submitted to transfer facility ownership/operation to another entity:
- General Application for Permit (LPC-PA1)
- Application for the Transfer of Solid Waste Permits (LPC-PA7)
- Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
- 39(i) Certification (for operating a waste management facility)
The following forms must be submitted to certify closure of the facility
The following forms must be submitted to certify completion of post-closure care of a facility
