Remediation System Information

A copy of a bid or an invoice for the remediation system from the manufacturer of the remediation system. If the bid or invoice does not contain the technical specifications of the remediation system, additional documentation must be submitted outlining the components of the system as well as individual costs of the components; or
If the remediation system will not be purchased from a manufacturer, the total cost to construct the remediation system must be provided. In addition, an itemized cost breakdown of the materials and subcontractors used and the personnel required to assemble the remediation system must be provided, including a list of each person's job title, the responsibilities, time spent, and rate for each job title. The specifications associated with the remediation system components and documentation of the costs associated in purchasing each part should also be provided.

The expected life of the remediation system and the disposition of all components of the remediation system once the remediation system reaches the end of the remediation project. In addition, a reasonable salvage value of the remediation system must be provided.

An estimate for a lease of the remediation system between the owner or operator and a lessor. A copy of the proposed lease between the owner or operator and the lessor associated with the remediation system should be submitted to the Illinois EPA. In addition, an explanation should be provided as to how the proposed lease may be terminated prior to, or extended beyond, the terms of the proposed lease agreement.

A breakdown of the items and activities that are specifically included in the proposed lease.

A breakdown of the items and activities that are not included in the proposed lease agreement and are necessary for the operation of the remediation system.

If the proposed lease includes costs associated with site-specific design of the remediation system, a cost breakdown for site-specific system design must be provided. This breakdown must include the personnel required to design the remediation system, including a list of each person's job title, responsibilities, time spent, and rate for each job title.

If the proposed lease includes costs associated with the design of the recovery portion of the remediation system (e.g., location of recovery trenches, recovery sumps or wells, extraction or injection points), a cost breakdown of these activities must be provided. This breakdown must include the personnel required to design the remediation system, including a list of each person's job title, responsibilities, time spent, and rate for each job title.

If the proposed lease covers cost for the replacement or repair of broken equipment or regular maintenance equipment (e.g., filters, carbon, chemicals) associated with the remediation system, the following items must be addressed:

- A list of all parts covered by the proposed lease;
- The estimated cost to replace or repair these parts over the lifetime of the proposed lease;
- The failure rate for these parts; and
- Written documentation on how the failure rate and cost to replace or repair were determined.

A list of any additional costs that may be included in the proposed lease rate including supporting documentation (i.e., receipts and invoices).
