Operating Landfills Under 35 IAC 807
(Landfills that initiated closure before September 18, 1992)
Note: All of these facilities already have a permit.
The following form is required to be submitted along with all permit applications:
The following forms are also required to modify a permit
The following forms must also be submitted to transfer facility ownership/operation to another entity:
- General Application for Permit (LPC-PA1)
- Application for the Transfer of Solid Waste Permits (LPC-PA7)
- Signatures of all Beneficiaries of a Land Trust (LPC-PA9) (if needed)
- 39(i) Certification (for operating a waste management facility)
The following forms must also be submitted to certify closure of the facility
